During the COVID-19 pandemic, OMC providers are still taking care of our patients. We are offering telemedicine to patients via desktop with camera or smart phones. If you already have an in-office appointment scheduled you may call your provider to switch your appointment to a telemedicine appointment. If you are a new patient, please complete this form after scheduling an appointment. Established patients do not need to complete these forms, but still need to call to schedule.
