Travel I-5 North and take Exit 81

Turn left, at the end of the ramp onto Mellen Street.

Follow this road, which turns into Cooks Hill Road.

Travel across the bridge and around the bend.

On your left you will see the Centralia Medical Center
where our offices are located.

From I-5, take exit 104 and drive west on U.S. 101 to the Black Lake
Blvd./West Olympia exit.

From the West Olympia exit of U.S. 101, turn right on Black Lake Blvd.
and prepare for a left turn at the first traffic signal.

Turn left on Cooper Point Rd. and drive .5 mile north to the
intersection with Capital Mall Drive.

Turn left on Capital Mall Drive and go west about .4 mile.

Make a right into the Columbia Capital Medical Center main entrance.

We are located on the third floor of the Physicians' Pavilion which is
directly attached to Capital Medical Center.
